SUMMARY: The CARE Center Intake Navigator II performs various
duties relative to the Intake/discharge and navigation. Primarily
they are responsible for the ongoing assessment of clients with
continued follow up to be sure he/she reaches their individual
goals and exit to permanent housing. This role will work with
Pinellas Homeless Management Information System (PHMIS) to open and
close all client cases, provide goal setting and assessment of
client needs. Overnight Navigators admit up to 120 homeless men and
women who sleep 70 in the Food Center dining area-turned
"dormitory" and 50 in outside Courtyard area. ESSENTIAL DUTIES AND
RESPONSIBILITIES : (These essential job functions are not to be

job at this time) Register all new clients between 2:30 p.m. and
6:00 p.m., using intake folders that include the individual's
demographics, income status, and other information about their
housing Set attainable goals and monitor progress of clients to
report to our Case Manager Provide assistance with the management
of client volunteers Routine monitoring of cleanliness in dining
area/lounge Assist clients with varying needs, i.e. general
information, housing, apartment searching, veteran assistance, and
referrals Provide assistance with restroom access, showers, bus
passes, environmental respite and use of a phone Use of Pinellas
Homeless Management Information System (PHMIS) Follow up and
evaluation on progress of clients then report to management on
client progression Work with volunteers to make sure showers and
laundry areas are functional and clean Scan clients into PHMIS for
all meal services as necessary Making ID badges for new clients and
replacements as needed Facilitate life-skill groups Knowledge of
harm reduction, motivational interviewing and trauma-informed care.
Ability to focus on moving client into Permanent Housing develop a
Housing Plan with clients Complete necessary paperwork including
incident reports, maintenance requests, and contact supervisor for
all necessary instances Oversee Scanning ID’s for all clients at
meal services Making ID badges Facilitate life-skill groups OTHER
RESPONSIBILITIES : Complies with all applicable training
requirements Complies with all company safety, personnel and
operational policies and procedures Complies with work schedule to
ensure effective operations of Agency programs. Contributes
positively as a member of a productive and cooperative team

to adapt to a fast pace and changing environment GENERAL PHYSICAL
DEMANDS : These physical demands are representative of the physical
requirements necessary for an employee to successfully perform the

requires prolonged sitting at the computer workstation; standing,
bending, reaching, lifting up to 20 lbs. and some driving. Requires

listen, the employee is also required to walk, use hands and
fingers to type, operate equipment, and maintain records and notes.

Environment will occasionally become noisy due to equipment
operations and interactions among clients and staff. There may be
the possibility of being exposed to communicable disease, possible
exposure to verbal abuse or similar behavior from
residents/clients. On an as needed basis, employees may be called
upon to work outside of the established work schedule, or work odd
